**About the Opportunity**
We're building a team of deeply passionate individuals who are ready to change primary care operations for those who need it most. As an Operations Support Associate, you'll play a critical role in ensuring our Care Operations team is set up to support our patients. You'll own completing our backend operational support processes, ensuring patient ECM and CHW program authorization and continued enrollment, all while creating a foundation of process efficiency and improvement.
**What You'll Do**
As an Operations Support Associate, you'll be responsible for:
* **Owning key Ops Support initiatives**: You'll take ownership of key operational support initiatives and ensure they're completed on time and to a high standard.
* **Proactively monitoring and reporting on key operational support processes and metrics**: You'll monitor and report on key operational support processes and metrics, identifying areas for improvement and proposing solutions.
* **Supporting a team of documentation analysts**: You'll troubleshoot and monitor project progress, providing support to a team of documentation analysts as needed.
* **Completing and monitoring ECM and CHW program authorizations and annual reauthorizations**: You'll ensure patient ECM and CHW program authorization and continued enrollment, completing and monitoring annual reauthorizations as needed.
* **Working with partnered health plans**: You'll work with partnered health plans to understand program denials and flag any enrollment issues.
* **Providing backend support for transitions of care and referrals**: You'll provide backend support for transitions of care and referrals, ensuring closed-loop follow-up from the care team.
* **Providing clear, timely communication to our care team**: You'll provide clear, timely communication to our care team when there's an enrollment change or status update.
* **Supporting Care Operations leaders**: You'll support Care Operations leaders in owning and optimizing key standard processes, becoming efficient in your role and proposing ways to improve.
**What You'll Need**
To be successful in this role, you'll need:
