Be the first point of contact for patients, families, and healthcare providers! Coordinate referrals, enter accurate patient data, and ensure smooth admissions for home health, hospice, and infusion services. Responsibilities
Coordinate referrals, enter patient information in the EMR, and ensure smooth patient admissions.
Serve as a liaison with hospitals, physicians, patients, and families to support timely, high-quality care.
Manage provider orders, documentation, and compliance to maintain accurate, efficient admissions.
Work in a fast-paced, collaborative environment where your contributions directly impact patient care.
Grow your expertise across Home Health, Hospice, and Infusion services while making a meaningful difference.
Qualifications
Education:
High school diploma or GED
Experience:
Required: At least 6 months of phone-based customer service experience in a fast-paced, high-volume environment, such as a call center.
Preferred: 1+ years of experience in home health or home medical equipment settings
