Healthspan Partners (HSP) is seeking an autonomous, deeply connected senior services professional to serve as Dementia Care Network Developer/Care Navigator. HSP is a remote, comprehensive dementia care model founded on the newly launched Medicare GUIDE Dementia Program.
This multi-faceted role is ideal for an entrepreneurial individual with a very established footprint in their local senior care sector. This dual developer/navigator role will introduce families and senior service organizations to the GUIDE benefit, recruit regional network partners, conduct initial intake assessments, and provide care navigation for their hub's initial cohort of families.
Key Responsibilities
• Network Development: Identify, engage, and recruit diverse senior healthcare and community partners to build out the local HSP GUIDE network. Target partners include:
• Home Health Agencies and Private Duty Home Care
• Physical Therapy and Occupational Therapy (PT/OT) providers
• Senior Living Communities (Independent, Assisted, and Memory Care)
• Local Health Care Systems and Provider Practices
• Dementia Support Groups and Community-Based Organizations (CBOs)
• GUIDE Benefit Introduction: Act as the local expert to introduce families, caregivers, and senior service organizations to the Medicare GUIDE benefit.
• Intake & Assessment: Conduct intake assessments for patients and their caregivers.
• Care Navigation: Serve as the dedicated Care Navigator for your hub's initial cohort, providing remote coordination, resource connection, and ongoing support for caregivers.
Critical Qualifications
• Dementia Care Expertise: Extensive experience with, and a deep dedication to, serving individuals living with dementia and their caregivers.
• Community Integration: Deeply embedded within your local community’s senior services sector with an active network of existing professional relationships.
• Relevant Professional Background: Minimum of 5 years of experience in at least one of the following areas:
• Social Work (SW) or Case Management
• Home Health or Home-Based Medical Care
• Community-Based Organizations (CBOs) serving high-needs older adults
• Senior care sales, relations, or business development with a strong clinical/social service understanding
• Preferred Credentials: Active RN, LPN, SW, OT, or NP licensure is highly preferred but not required for candidates with exceptional relevant experience.
Pay: $2,000.00 - $8,000.00 per month
