About the Role
Pathfinder Care, based in St. Paul MN, is seeking a Licensed Alcohol and Drug Counselor (LADC) to provide high-quality, timely substance use assessments across Minnesota. This role is responsible for completing comprehensive assessments, documenting findings, and ensuring appropriate referrals and follow-up to support continuity of care. Assessors play a critical role in access to services and are expected to maintain accuracy, timeliness, and compliance with Minnesota DHS, HIPAA, and 42 CFR Part 2 requirements.
Key Responsibilities
Assessment & Documentation
• Complete comprehensive substance use assessments
• Ensure all assessments are: Clinically sound, fully documented and completed within required timelines (target: within 24 hours of service)
• Accurately document medical necessity and level of care recommendations
Referral & Follow-Up
• Initiate and document outreach attempts to referred clients in a timely manner
• Provide appropriate referrals based on assessment outcomes
• Follow up on referrals and collateral when applicable
• Maintain clear and accurate documentation of: Contact attempts, client engagement, and referral outcomes
Workflow & Communication
• Manage assigned assessments and timely referrals
• Utilize Pathfinder systems (Acuity, EHR, phone system) for scheduling, documentation,
• and communication
• Communicate effectively with internal staff and external referral sources
• Ensure all client interactions are professional, timely, and documented
Compliance & Quality
• Adhere to: Minnesota DHS requirements: HIPAA and 42 CFR Part 2 regulations, Organizational policies and procedures
• Maintain accurate, audit-ready documentation at all times
• Participate in quality assurance and audit processes as needed
Minimum Qualifications
• 18 years of age.
• Must be free of chemical use problems for at least the two years immediately preceding hire.
• Must have or obtain 12 hours of training in co-occurring mental health problems training within six months of hire
• Ability to provide individual crisis intervention.
• Must be licensed or exempt from licensure under Minnesota Statutes 245G.
• Must document competence in screening for and working with clients with mental health problems, through education, training, and experience.
• Knowledge and understanding of the implication of this chapter, sections 626.557, and 626.5572, and chapters 254A and 260E.
• Ability to successfully complete MN Department of Human Services background study.
Preferred Qualifications
• Experience completing Rule 25 / comprehensive assessments
• Familiarity with ASAM criteria and level of care placement
• Experience working with Medicaid/PMAP populations
• Strong documentation and time management skills

• Ensure no referral is left without documented follow-up
• Maintain accurate and truthful reporting of all client contact attempts
Work Structure
• 1099 independent contractor role
• Flexible scheduling based on availability and referral volume

• Pay is based upon each completed assessment.
