We are seeking an experienced healthcare operations leader to help architect and operationalize that future.
Position Summary
The Vice President of HealthBridge Operations & Clinical Integration will lead the development, standardization, implementation, and scaling of the HealthBridge service delivery model across hospital and healthcare system partners nationwide.
This executive leader will transform successful field-based practices into a repeatable, measurable, and scalable national operating system. The ideal candidate combines deep healthcare operational expertise with strong process design, care transition knowledge, and multi-site implementation experience.
This role is ideal for a builder—someone energized by creating structure, designing scalable workflows, implementing operational rigor, and leading cross-functional healthcare execution in a high-growth environment.
This is not a traditional sales role.
This is a healthcare service-line development and operational leadership role.
Key Responsibilities
Operational Strategy & Service-Line DevelopmentDesign and operationalize the national HealthBridge care transition delivery model
Translate successful field-based practices into standardized workflows, SOPs, and scalable operational processes
Develop implementation playbooks for hospital and health system partnerships
Build operational infrastructure capable of supporting multi-market expansion
Hospital & Healthcare System IntegrationPartner with hospital leadership, case management teams, discharge planners, and healthcare stakeholders to support successful integration
Establish standardized onboarding and implementation protocols for new health system partners
Develop escalation pathways and service recovery procedures
Ensure consistent operational execution across all markets
Clinical & Care Transition OperationsCreate standardized best practices for patient navigation, discharge coordination, and post-acute transition support
Ensure workflows align with hospital throughput goals, patient experience expectations, and quality metrics
Collaborate with clinical and operational stakeholders to improve continuity of care and reduce transition friction
KPI Development & Outcomes ReportingEstablish operational KPIs and service-level metrics
Develop dashboards and reporting frameworks for hospital partners and executive leadership
Monitor program effectiveness, operational consistency, referral responsiveness, and transition outcomes
Drive continuous process improvement initiatives
Training & Field EnablementBuild and oversee HealthBridge operational training and certification programs
Develop implementation resources, communication protocols, and operational standards for field teams
Partner with franchise and regional leadership to support execution consistency
Cross-Functional LeadershipCollaborate closely with executive leadership, business development, franchise operations, technology, and clinical stakeholders
Serve as a strategic operational leader during HealthBridge national expansion
Help shape long-term organizational strategy and scalable infrastructure
Qualifications
Required Experience10+ years of progressive leadership experience in healthcare operations, care transitions, population health, post-acute care, or hospital operations
Experience building or scaling multi-site healthcare operational programs
Strong understanding of hospital discharge planning, case management, care coordination, and post-acute ecosystems
Proven success creating standardized workflows, SOPs, operational playbooks, and scalable service models
Experience leading implementation initiatives across multiple markets or healthcare systems
Strong analytical and KPI-driven operational management experience
Preferred ExperienceExperience working with hospitals, health systems, ACOs, Medicare Advantage organizations, or value-based care programs
Experience in transitional care management, throughput operations, population health, or post-acute strategy
Familiarity with franchise, distributed workforce, or multi-market operating environments
Experience in healthcare startups, growth-stage healthcare organizations, or healthcare services expansion
