About Us: Eag-Us is a multi-state mental health organization dedicated to improving quality of life and mental health outcomes for older adults. We provide behavioral health services across skilled nursing facilities and long-term care settings, combining in-person clinical care with telehealth options as appropriate. Position overview: We are seeking a traveling Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) to deliver compassionate, evidence-based clinical services to elderly residents in nursing homes and other long-term care settings across assigned regions. The ideal candidate will combine strong clinical skills with flexibility for travel and facility-based work. • Primary responsibilities • Conduct clinical assessments, diagnostic evaluations, and ongoing mental health treatment for older adults in nursing homes and long-term care facilities. • Develop, implement, and revise individualized treatment plans and goals, using culturally competent and trauma-informed approaches. • Provide individual and group therapy, crisis intervention, behavioral management, and family/caregiver consultations as needed. • Coordinate care with interdisciplinary teams including nursing staff, case managers, physicians, and facility administrators to support continuity of care. • Maintain timely, accurate clinical documentation in electronic medical records (EMR) consistent with professional and regulatory standards (HIPAA compliance). • Participate in facility credentialing and quality-improvement activities; support program development for geriatric behavioral health services. • Utilize telehealth when appropriate to supplement in-person visits and extend access to care. • Required qualifications • Active, unrestricted LPC or LCSW license in the state(s) of practice, or demonstrated ability to obtain required state licensure for assigned regions. • Clinical experience working with older adults, geriatric populations, or within nursing homes, skilled nursing facilities (SNF), or long-term care settings. • Demonstrated competence in assessment, treatment planning, psychotherapy, and crisis management for geriatric mental health concerns (e.g., depression, anxiety, dementia-related behavior). • Proficiency with electronic medical records (EMR) and documentation practices; strong written and verbal communication skills. • Ability and willingness to travel regularly to multiple facility locations; valid driver’s license and reliable transportation (or willingness to use company-provided travel arrangements where available). • Ability to meet facility credentialing requirements (background check, vaccinations/TB screening, and any facility-specific clearances). • Preferred qualifications / nice-to-haves • Certification or formal training in geriatric mental health, dementia care, or palliative care; experience with behavioral interventions for dementia. • Experience with Medicaid/Medicare documentation processes and working within interdisciplinary long-term care teams. • Training in evidence-based modalities such as CBT, Motivational Interviewing, or trauma-informed care. • Bilingual proficiency or experience serving diverse cultural populations. • Telehealth delivery experience and comfort using remote platforms. • Benefits and working conditions • Competitive compensation structure tailored to traveling clinicians. • Mileage reimbursement or travel support for facility visits; flexible scheduling to balance travel and clinical time. • Opportunities for continuing education, supervision, and professional development; support for license renewal where applicable. • Access to telehealth platforms to supplement in-person care and increase caseload flexibility. • Work focused on high-impact geriatric populations with opportunity to influence care models across multiple facilities.
