Key Responsibilities
• Conduct comprehensive virtual consultations for patients in post-acute care facilities across Florida.
• Diagnose, treat, and manage a wide spectrum of endocrine conditions, including diabetes, thyroid disorders, adrenal and pituitary diseases, and osteoporosis.
• Develop and adjust evidence-based treatment plans, including prescribing medications, insulin management, and other therapeutic interventions.
• Utilize telehealth platforms, EMRs, and remote monitoring devices (e.g., continuous glucose monitors) for ongoing care and treatment adjustments.
• Collaborate closely with on-site nurses, facility medical directors, and primary care providers to ensure seamless transitions of care and effective endocrine management.
• Provide clinical leadership and guidance for endocrine care programs within partner facilities, promoting best practices and optimizing patient outcomes.
• Ensure meticulous and timely documentation of all patient encounters within the EMR/EHR system in compliance with all relevant Florida and federal regulations.
• Educate facility staff on best practices for endocrine care, including prevention strategies, treatment adherence, and patient education.

Qualifications & Requirements
• Active Florida License: Must hold a current, unrestricted NP or PA license in the State of Florida (a dealbreaker!).
• Education: Master’s or Doctorate degree in Nursing (MSN/DNP) or Physician Assistant Studies (MPAS) with a specialization or relevant experience in Endocrinology.
• Experience: Minimum of 2 years of clinical experience in endocrinology. Previous experience in post-acute care, skilled nursing facilities, or long-term care environments is highly preferred.
• Proof of Liability Insurance: Post Acute Specialists provides insurance coverage for Nurse Practitioners and Physician Assistants as part of the position benefits, despite the 1099 status.
• Technical Skills: Proficiency in EMR navigation (experience with common SNF EMR systems is a plus) and comfort utilizing telemedicine technology.
• Skills: Strong commitment to quality improvement, excellent communication skills for collaborating with diverse facility staff, ability to work autonomously, and clinical empathy tailored to the geriatric and post-acute population.
