Company Overview
We are a licensed pediatric home care agency in Georgia serving medically fragile children through the Georgia Pediatric Program (GAPP) — a Medicaid waiver that provides skilled nursing and personal care to children under 21 at no cost to families. We are growing and looking for a dedicated remote coordinator to help us connect more families with the life-changing care they need.
Position Summary
This is a remote, part-time role (20-25 hours per week) focused on outreach to medical referral sources, intake coordination, and follow-up with families. You will be the first point of contact for referral partners and families, and will manage all activity inside our Go High Level CRM. This role is critical to our growth — your success directly determines how many children we can serve.
Key Responsibilities
• Make 10-15 outbound calls per day to pediatric offices, hospital discharge planners, case managers, school district nurses, and community health workers across our Georgia service counties
• Build and maintain relationships with referral sources through consistent weekly follow-up
• Conduct warm intake calls with families referred to our agency and guide them through the GAPP enrollment process
• Log all calls, contacts, and referrals in Go High Level CRM same-day
• Send follow-up texts and emails to unresponsive families (minimum 3 touch attempts per referral)
• Maintain a master referral source database organized by county and source type
• Track and report weekly metrics to agency owners
• Learn the GAPP program basics to answer common family questions confidently
• Schedule intake appointments and coordinate paperwork collection
Requirements
• Fluent English — clear, professional phone presence is essential
• Previous experience in healthcare, home care, medical office, or patient coordination preferred
• Comfortable making high volumes of outbound calls daily
• Experience with a CRM (Go High Level preferred, or similar)
• Self-motivated and able to work independently with minimal supervision
• Reliable internet connection and quiet workspace
• Available during US Eastern Time business hours (9am-5pm ET, Monday-Friday)
Compensation
• Base pay: $8-$15/hour USD (negotiable based on experience)
• Performance bonus: $50 per qualified referral that is submitted to Medicaid
• Additional $75 bonus per patient approved and enrolled
• Hours: 20-25 hours per week to start, with potential to grow to full-time
