Apply Now
About the Role Flexible telehealth schedule. A physician team that actually implements your nutrition plans. Patients who follow through. At Avita MD, nutrition isn’t an afterthought — it’s integrated into every patient’s care plan. Our CEO is double board-certified in Internal Medicine and Obesity Medicine, and our precision medicine programs use lab-driven data (metabolic panels, hormone profiles, inflammatory markers) to personalize treatment. You’ll be the dietitian who gets the referrals, the labs, and the physician partnership that makes your work effective. This is a part-time telehealth position (1–2 days per week) embedded within a growing, physician-owned primary care practice serving patients across Los Altos, Mountain View, and Los Gatos. Our patient panel is predominantly PPO-insured professionals and families in Silicon Valley who are motivated, health-literate, and engaged in their care. What You’ll Do Medical Nutrition Therapy • Conduct comprehensive nutrition assessments for patients with diabetes, prediabetes, dyslipidemia, hypertension, NAFLD/MASLD, GI conditions, and other chronic diseases • Develop individualized nutrition care plans coordinated with the patient’s physician and care team • Provide follow-up counseling and track patient outcomes via our EHR (Elation) Weight Management & Obesity Medicine Support • Partner with our obesity medicine–certified physician on comprehensive weight management programs, including patients on GLP-1 agonists (semaglutide, tirzepatide) and other pharmacotherapy • Design structured meal plans and behavioral nutrition interventions for patients at various stages of their weight loss journey • Support patients through metabolic adaptation, maintenance phases, and long-term lifestyle change Functional & Precision Nutrition • Interpret lab results (metabolic panels, micronutrient levels, inflammatory markers, hormone panels) alongside the physician team to develop nutrition protocols • Address root-cause nutritional contributors to fatigue, brain fog, hormonal imbalances, and gut health issues • Educate patients on evidence-based supplementation, elimination diets, and anti-inflammatory nutrition strategies What We’re Looking For Required: • Registered Dietitian Nutritionist (RDN) credential with active CDR registration • Active NPI and CAQH ProView profile, or willingness to obtain prior to start date • Minimum 5 years of clinical nutrition experience in an outpatient or primary care setting • Proficiency with telehealth platforms and electronic health records • Comfortable with a patient population that asks hard questions and does their own research Preferred: • Board Certification in Obesity and Weight Management (CSOWM) or Certified Diabetes Care and Education Specialist (CDCES) • Experience supporting patients on GLP-1 receptor agonist therapy • Familiarity with functional medicine frameworks and advanced lab interpretation • Experience with Elation EHR (a plus, not required) • Multilingual candidates are a plus (our patient population includes Russian- and Spanish-speaking families) Why Avita MD • Physician-owned, not corporate: Our CEO practices medicine alongside the team. Clinical decisions are made by clinicians, not administrators. • Your recommendations get implemented: Direct collaboration with an obesity medicine–certified physician means your nutrition plans are reinforced in every medical visit, not filed away. • Engaged, motivated patients: Our Silicon Valley patient panel is health-literate, invested in outcomes, and follows through. These are the patients dietitians wish they always had. • Flexibility: Fully remote via telehealth with scheduling flexibility. Build this around your existing commitments. • Growing practice: 3 locations today and continuing to expand. As patient volume grows, so does the opportunity for this role to expand.
