Per Diem Physical Therapist (PT)

Overview:
We are a certified home health agency seeking a compassionate and experienced Per Diem Physical Therapist (PT) to provide skilled therapy services to patients in their homes. As a key member of the interdisciplinary team, the PT will evaluate patient needs, develop individualized treatment plans, and implement evidence-based interventions to help patients regain mobility, improve function, and achieve optimal outcomes.

Responsibilities:
• Conduct in-home physical therapy evaluations and reassessments in accordance with the physician's plan of care and Medicare guidelines.
• Develop and implement individualized therapy plans to restore function, improve mobility, relieve pain, and prevent physical disability.
• Instruct patients and caregivers in safe use of assistive devices, exercises, and therapeutic techniques to support continued progress.
• Monitor and document patient response to treatment and modify plans as needed to achieve therapy goals.
• Collaborate with the interdisciplinary care team (nurses, occupational therapists, speech therapists, aides, and physicians) to ensure coordinated and effective care.
• Maintain timely, accurate, and compliant documentation in the agency’s EMR system.
• Travel to patient homes throughout the assigned territory
Schedule & Compensation:
• Per Diem / PRN – Flexible schedule based on your availability and patient needs.
• Competitive per-visit rates for initial evaluations, follow-up visits, and reassessments.
• Current Physical Therapist license in the State of Florida.
• Minimum of one (1) year of experience as a licensed PT; home health experience strongly preferred.
• Thorough understanding of Medicare documentation requirements and therapy goals in the home health setting.
• Proficiency in EMR systems (experience with Kinnser/WellSky or similar is a plus).
• Excellent interpersonal, communication, and time management skills.
• Ability to work independently and manage a flexible per diem schedule.
• Reliable transportation, valid driver’s license, and current auto insurance.
