Job Overview
Join our dynamic care team as a Remote Care Coach, RN (Registered Nurse) or LVN (Licensed Vocational Nurse), where you will play a vital role in delivering exceptional patient-centered virtual care from the comfort of your home. In this energetic and impactful position, you will utilize your clinical expertise to assess, educate, and support patients across diverse settings, including remote physiologic and remote therapeutic monitoring, principal and chronic care management. Your proactive approach will help improve health outcomes, promote patient engagement, and streamline care coordination through effective communication and utilization management. Ideal Candidate:
A Care Coach is responsible for the success of our white-glove virtual care program. You will be the primary point of contact that patients have with our company and will operate as an extension to one or more of our customer clinics. The ideal candidate demonstrates a strong enthusiasm for patient care. This role requires a highly empathic individual who is a strong communicator and has an exceptional in-person and on-phone demeanor. Candidate Requirements:
• REQUIRED: Registered Nurse (RN) or Licensed Vocational Nurse (LVN)
• REQUIRED: Must be fluent in written and spoken Spanish and English
• HOURS: 40 hours a week, work week is Wednesday - Sunday.
• At least 1 year of patient care experience with demonstrated ability to effectively engage and communicate with patients, relevant clinical training, or academic experience will be considered
• Chronic Care Management experience (preferred)
• Experience conducting Patient Assessments/Wellness Exams (preferred)
• Ability to discuss financial implications with healthcare coverage (e.g. possibilities of co-pay obligations, etc.)
• Ability to clearly articulate to patients how to use the MiCare App and connected devices
• Ability to communicate clearly with all age populations and of various medical conditions, including those with hearing and cognitive challenges
• EMR charting experience
• Time management/organizational skills
• General computer skills, including Email, Word, Excel, and Patient/Medical Systems
• Excellent listening skills and phone presence
• Remote work experience is a plus. The candidate must have a conducive work environment and high-speed Internet for remote work
• Orthopedic, osteoporosis, and cardiology care experience is a plus
• Ability to manage patients at varying times in their care journey

Key Responsibilities:
• Working collaboratively with patients to determine their medical needs and goals, develop the best course of action, and oversee, maintain, and manage their treatment plans and progress towards goals, ensuring each client gets high-quality, individualized care
• Conduct a high volume of patient outreach calls and messages in support of patient engagement and technical support
• Oversee a monthly patient roster, ensuring comprehensive care for each patient
• Remotely monitor the patient’s daily physiologic and patient-reported measures using the MiCare platform
• Aiming for a patient engagement rate of 90% or higher
• Critically think to continuously improve our processes and procedures, identify and prioritize systems/workflow enhancements that will make onboarding easier, more efficient, and faster for patients.
• Assist MiCare Path’s Patient Team with daily administrative tasks, including facilitating shipment of necessary medical devices, troubleshooting the devices and app with the patient, reporting, etc.
• Potentially work/travel between 1-2 Specialty Clinics, educating and facilitating patient enrollment in the MiCare Path program
• Collaborate with physicians, healthcare staff, patients, families, and the MiCare Path team
Pay: $25.00 - $30.00 per hour
